- Proof of birth: Birth certificate, passport, or ID. We only need to see it at check-in — copies aren't collected until an offer is accepted.

- CHRVA MEMBERSHIP: Proof of current membership required at check-in. (you can see how to access your digital card here)

- USAV Player Medical Release Form (access here)

*Required Documents Must Be Submitted Before Tryouts. Incomplete or missing documentation will prevent or delay participation in tryouts.
